From what you described in your question your dog is not getting a proper diet. In your question you did not mention a source of calcium, this is very important to balance the phosphorus in the meat and this could be contributing to your dog's liver and kidney issues. For your vet to say" keep doing what you are doing" is very strange with your dog doing so poorly.
