Whether to provide a dog with heartworm medication is a personal choice and also dependant on location. Here in Connecticut we have not used heartworm meds for our dogs and have not had an issue in twenty years. However, if you live in a more heartworm endemic area like the southeast or gulf coast, using heartworm medication takes on more importance. Heartworm medications seem to be of less less concern than flea and tick medications/collars for the risk posed to the nervous system of dogs with, or at risk for, degenerative myelopathy.
